FTA: The Foster-to-Adopt Program is to provide the fostering party a limited time period, to determine if the chosen pet is a good fit for their home, pets, kids..etc.

FOSTER: The Foster Program is an important part of our Rescue. We are a ‘Home Foster Based’ Rescue, and therefore rely on Fosters to temporary house a pet, until a permanent home is found.
*Fosters have the option to adopt their foster pet (approval of application, agreement and Fee is required)

Failure to attach your DL and falsifying age on any document will Void all your applications and agreements, and will be considered a *Breach of Contract.

I certify that I, the fostering party, will be the main caretaker of the pet and will not give the pet to any other persons, boarding facility or organization at any time, unless I have written approval from MHR.

Fostering party agrees to notify MHR immediately if they notice any changes in the dog, such as, but not limited to; is injured in any way, looks sick, stops eating, has noticeable weight loss, change in personality, change in feces/diarrhea, has worms, rashes, fleas, seems lethargic, or anything out of the ordinary. DO NOT take the dog(s) to seek medical attention, until you have made MHR aware and have received further instructions. We will most likely send you to our closes Veterinary office.

If Fostering party, for whatever reason, can no longer care for said dog, or if a decision has been made to not adopt the pet, the fostering party agrees to notify MHR immediately and will not under any circumstance attempt to rehome the dog themselves. Fostering party Must give ample notice to give MHR time to find another home for the dog (a few days/weeks are required).

I agree that I will return the pet at any time that I am requested to, and will not under any circumstance hold the dog for any reason whatsoever.

Fostering party agrees to keep any puppy or non-fully vaccinated adult dog, away from public places. This includes, but is not limited to: your front yard, your backyard, parks, beaches, hiking trails, baseball fields, neighborhood walks, store visits, other family home visits, public sidewalks, and away from other unvaccinated dogs/puppies… etc.

I agree that I will Not rehome, adopt out, give to, give away, sell, advertise online, or relinquish the pet to anyone, under any circumstance, at any time.

I acknowledge the following:
**Breach of Contract: Failure to comply with any and all provisions, instructions and requirements listed in this agreement, unless otherwise agreed to in writing by MHR, will be considered a Breach of Contract. The Fostering party will be required to, and is agreeing to, be responsible to return the pet(s) to MHR immediately, at their own expense, time and resources.

I acknowledge that this agreement and anything listed hereto, is Not all inclusive. MHR reserves the right to remove pet(s) at any time deemed necessary by MHR, for any reason not disclosed to fostering party, and without any explanation.

I agree that if the dog(s) in my care is due for vaccines and I do not comply or make myself available, and the vaccine goes past it's due date, I am responsible to cover the cost of the missed vaccine, as well as any prior vaccines in that set, should the set need to start over.

**FTA Applicant Only: After the designated foster-to-adopt period, the fostering party agrees to make the determination if they would like to officially Adopt or Return the foster pet, In Writing to MHR, NO LATER THAN 8 DAYS from the day they received the foster pet.

-IF ADOPTING: Adopter agrees to submit the ADOPTION FEE, ADOPTION AGREEMENT AND ANY OTHER DOCUMENTS SENT TO APPLICANT, and return all documents to MHR, NO LATER THAN 2 DAYS after the end of your FTA period (DUE ON THE 10th DAY).

IF THE APPLICANT DOES NOT SUBMIT THE ABOVE DOCUMENTS AND ADOPTION FEE BY THE ALOTED TIMEFRAME, MHR HAS THE RIGHT TO REMOVE THE PET(S) AT ANY TIME, WITOUT NOTICE. THE APPLICANT IS OBLIGATED TO TRANSPORT PET(S) TO AN MHR DESIGNATED LOCATION OR BE AVAILABLE FOR MHR TO PICK UP PET(S) AT ANY TIME.

IT IS THE APPLICANTS RESPONSIBILITY TO KEEP TRACK OF THE FTA DAYS. IT IS THE APPLICANTS RESPONSIBILITY TO CONTACT MHR ON THOSE DUE DAYS. IF YOU DO NOT CONTACT MHR OR YOU GO PAST THE ALOTED DAYS ABOVE, YOU ARE IN BREACH OF CONTRACT.

I, the Fostering party, agrees that MHR makes no express or implied warranty, promise, or representation as to age, health, breed, habits, disposition or safety of the animal. I hereby accept the animal As Is, assume all risks and responsibilities associated with ownership, temporary housing, and possession of the pet(s), including but not limited to; bites, contagious illness, parasites..etc., and hereby fully and completely release, indemnify and hold harmless MHR, its directors, officers, servants, volunteers and employees from any claim, cause of action, liability of any nature, known or unknown, directly or indirectly arising out of or in connection with adoption, care, ownership, condition, maintenance or temperament of pet(s). I acknowledge that I have read this agreement in it’s entirety, fully understand all terms and conditions, and I will fully comply. Failure to comply with any provision listed, will be considered **Breach of Contract. If it becomes necessary to involve legal representation for any reason, Fostering party agrees to pay all attorney's fees necessary as permitted by law, including filing fees, incurred by MHR.
