This contract allows the Adopter, named above, to care for the PPR-owned Dog named above, with the intent that ownership of the dog will transfer to the Adopter when the dog is neutered/spayed before he/she reaches the age of 28 weeks (see Neuter/Spay Addendum, Question 7 below). The Adopter has paid Poodle and Pooch Rescue (PPR) an adoption donation in consideration for this dog. PPR’s Vetting Coordinator will be in touch with Adopter to coordinate and/or schedule all follow-up vetting, including additional vaccines through spay/neuter. All appointments with PPR’s veterinarians must go through PPR’s Vetting Coordinator. Adopter is not to contact PPR vet directly; all vetting questions and scheduling must be communicated through the Vetting Coordinator: vetting@poodleandpoochrescue.org
1. I understand that this dog belongs to PPR and does not belong to me until and unless the dog is neutered/spayed before it reaches 28 weeks old. I understand and agree to have this dog neutered/spayed before it reaches 28 weeks old either by a PPR vet (at PPR’s expense) or my own vet (at my own expense). I understand that PPR will make an appointment for me to have the dog neutered/spayed at a PPR vet, but I can choose for my own vet to perform the neuter/spay before the dog reaches 28 weeks old. After this dog is neutered/spayed by a PPR vet or I present Proof of Neuter/Spay (as defined on page 2), the ownership of the dog transfers to me.
2. I will provide a safe, loving environment for this dog and treat this dog as a member of my family. I will provide proper nourishment and medical care including monthly heartworm, flea prevention and regular grooming. This dog will live inside my home, not outside.
3. I recognize that although PPR has made an effort to provide a dog that is compatible with my home environment, PPR cannot and does not warrant the temperament or behavior of this dog. I release PPR and its members from all liabilities or losses that may result from damage or injury resulting from this dog
4. PPR has advised me of any known health issues, however this dog could be carrying a virus or other illness that may be contagious to animals or humans. PPR strongly recommends that I take this dog to my veterinarian as soon as possible for a wellness exam and heartworm and flea prevention. This will not be reimbursed by PPR.
5. I will immediately notify PPR if this dog is lost or stolen or if I am unable to keep this dog. I won’t relinquish this dog to another party without the written consent of PPR. This dog will wear a collar with identification tags at all times, including a microchip tag.
6. PPR is authorized to remove this dog from my home if it is judged not to be receiving proper care or if there is a violation of this agreement. I agree to permit PPR to visit the home where the dog is kept given reasonable notice and that such entry shall not constitute a trespass.
7. I will contact PPR and/or accept PPR’s call or email to arrange for this dog, which is owned by PPR, to have additional vaccines and neuter/spay at a PPR veterinarian before this dog reaches 28 weeks old. PPR will schedule appointments for me. In the alternative, I will provide proof that my vet performed each set of vaccines after each appointment as well as neuter/spay and that each was done before the dog is 28 weeks old. “Proof of Neuter/Spay” consists of written veterinary records and the ability of a PPR representative to verbally confirm with my veterinary office. I understand that this dog is and remains the property of PPR and ownership only transfers to me when this dog is neutered/spayed before he/she reaches 28 weeks old at a PPR vet or if I provide Proof of Neuter /Spay before this dog reaches 28 weeks old. If PPR is unable to reach me by email or phone to schedule this neuter/spay, PPR will require that I return their dog back to them and I will. If the dog is not neutered/spayed by 28 weeks old, PPR will require that I return their dog back to them and I will. I understand that if I do not comply with PPR’s attempts to recover their dog from me, I will be in possession of stolen property.
8. I understand that PPR is financially responsible for all initial vaccines, spay/neuter and microchip (if completed with a PPR vet). I understand that I am financially responsible for starting the puppy on monthly preventative medications such as heart worm and flea prevention, as well as starting the dog on a healthy puppy food. Puppies should remain on a puppy diet as long as they’re still growing and shouldn’t transition to an adult diet until one year of age. I understand that I am responsible for any grooming and veterinary care outside of vaccines, spay/neuter.
9. This Agreement shall be governed by and construed in accordance with the laws of the State of Florida. The parties agree that the state and Federal courts located in Orange County, Florida shall have exclusive jurisdiction over any dispute arising in connection with this Agreement
